Community Grants Panel Member Registration

The Community Foundation for the CSRA is committed to maintaining the highest standards of impartiality and transparency in our grant review process. Our volunteers play an important role in upholding these standards.

To help ensure a fair and impartial review process, please disclose any nonprofit organizations with which you or an immediate family member have a current or recent personal, professional, volunteer, or financial connection that could create an actual or perceived conflict of interest.

Examples may include serving as a board or committee member, volunteering, current or previous employment, providing professional services, or another close affiliation with a nonprofit organization.

Please provide the name of the nonprofit organization and a brief description of your connection.

Disclosure of a relationship does not necessarily prevent you from serving on a panel. Foundation staff will use this information to identify potential conflicts, make appropriate panel assignments, and determine when a volunteer should refrain from reviewing or discussing a particular organization.

If you have no relationships to disclose, please enter “None.”
